Good health and wellness is an essential part of our lives. We offer completely confidential biometric screenings and healthy lifestyle support at no cost to you! This screening assesses your body mass index, blood pressure, cholesterol, and A1c blood sugar levels.
If you are in the Company’s medical plan, you and your covered spouse will be required to complete a biometric screening during our campaign cycles every other year. Employees and covered spouses completing a screening in 2018 will not be required to do another one until the 2020 campaign. If you or your covered spouse do not complete biometrics testing when it is required (during a campaign year), you will be charged an additional $28.85 per week or $125 per month as a surcharge on top of your medical plan premium.
If you or your covered spouse are selected for healthy lifestyle support based on your biometrics results, please engage with your Healthy Life Navigator to avoid a wellness surcharge of an additional $28.85 per week or $125 per month as a surcharge on top of your medical plan premium.

New Hires or New to the Plan with a Qualifying Life Event
You and your covered spouse must complete a biometric screening within 60 days of your benefit effective date. Employees and covered spouses who do not complete biometrics screening will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium. Healthy Lifestyle Support Required for Not Meeting Standards
Members not meeting the recommended biometric standards will be required to engage in healthly lifestyle support via face-to-face, telephonic, virtual or online learning to avoid a wellness surcharge. Members must meet all biometric standards to be considered passing. Please review the numbers (above right).
Where to Go to Complete Your Biometrics
For your convenience, we are offering many ways for you and your spouse to complete your biometric screening.
Healthy Life Centers (Spouses)
Utilize the Healthy Life Centers if there is one located nearby. Schedule an appointment by texting or calling 877-365-0051.
Bring to Your Appointment: Medical ID Card and ID (ex. Driver's license). Note: You do not need to bring a Quest Physician Form to the HLC for biometric screenings.
Quest Patient Service Centers
Employees and spouses may also choose to visit a Quest Patient Service Center. Appointments are not required, but pre-registration is. You may register by logging on to Quest at my.questforhealth.com. You will need your employee ID number and the registration key—Mohawk2019. Spouses will need to use the employee ID number followed by an “S” (ex: 30124s) to log on and schedule an appointment. If both members are employees at Mohawk, the spouse should use the employee ID (plus “s”) belonging to the primary carrier of the medical plan. You can rescreen every 3 months with Quest. If you haven’t already registered on the Quest website, you must be registered before you can complete a screening. You may also call Quest at 877-304-7055.
Bring to Your Appointment: ID (ex. Driver's license)
Employees and spouses may also choose to visit their personal physician. Biometrics data can be included as part of a routine annual physical. To complete your biometrics screening during a wellness visit with your doctor, you will need to register at my.questforhealth.com to be able to download the Quest Physician Results Form. The registration key is Mohawk2019. Complete the registration form as indicated. Spouse will need to register separately and use the employee ID number followed by an “S” (ex: 30124s). Once registered, you will need to select, “Get started,” and then, “Participate now.” The Physician Results Form is located in the bottom left section of the page. Here you can download and print a form pre-populated with your information. Take the form with you to your appointment for the physician office to complete.
For your test results to be counted, all fields on the form must be completed by both physician and member where indicated. Forms will be denied if incomplete. Completed forms should be faxed to Quest at the number on the form. If faxing multiple pages, please send one page per transmission with no cover page.

If you live and/or work greater than 50 miles from a Healthy Life Center or a Quest Diagnostics Patient Service Center, you may request a Home Biometric kit be sent to you by calling Quest at 877-304-7055.
If you have a medical condition that indicates your provider accepts a biometric measure as passing which does not meet Mohawk standards, be sure to bring the Physician Affidavit/Waiver Form below with you to the appointment. Once the exemption form has been submitted, the member should follow up with Quest to ensure the form has been received and processed.
If you are currently working with your personal physician on a wellness plan, use the Physician Health Coaching Engagement Waiver form below to waive out of coaching. Your provider must complete the entire form, sign off on your health improvement plan and indicate the status of your plan and sign the form. The completed forms may be faxed or emailed to the Benefits Service Center. Once the exemption form has been submitted, the member should follow up with BSC to ensure the form has been received and processed.
How Do I Get My Results?
Approximately 10–15 days after completing your biometrics, a letter with be mailed with the results.

Making lifestyle adjustments is hard on your own, but it’s easier with support. At Mohawk, you don’t have to face your wellness journey alone. We make it easy for you to be healthy and live better so you can be your best at home and work. Good health and wellness is an essential part of our lives. We offer completely confidential healthy lifestyle support as an enhancement to medical plan participants. Remember, making small changes makes a big difference over time.
How to Engage in Healthy Lifestyle Support
The Benefits Service Center or the Healthy Life Team will reach out to members required to engage in healthy lifestyle support. Members required to engage in healthy lifestyle support will receive various forms of communication including postcards, phone calls, emails and text messages. You may also contact the Benefits Service Center at 866-481-4922 if you are unsure if you or your spouse needs to engage.
Healthy Life Navigators are here to guide and motivate members to make healthy lifestyle changes. They will work with you or your spouse to set a personal health goal, and they will give you the tools to achieve or make progress toward that goal. Healthy Life Navigators build an essential foundation for self-management to help you to continue to use the right tools, behaviors, and resources in day-to-day life. And as always, the support Healthy Life Navigators provide is completely confidential and offered as an enhancement to the medical plan.
Employees and covered spouses who choose not to work with a Healthy Life Navigator, do not make contact with their Healthy Life Navigator or do not complete biometrics testing will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium.

How to Get a Wellness Surcharge Removed
You may receive a wellness surcharge for any of the following:
- No biometrics screening for employee and/or spouse
- No healthy lifestyle support engagement for employee and/or spouse
So how do you get a wellness surcharge removed?
- Complete a biometrics screening
- Engage with a Healthy Life Navigator or Healthy Life Center provider
- Call the Healthy Life Team at 855-566-4295 to begin working towards a goal
Once the completed screening or coaching has been processed, the surcharges will be removed. The surcharge removal process may take up to 60 days.
Special Notice to Critical Illness Plan Policy Holders
Critical Illness policy members (employee/spouse) can receive $75 for a health screening. Reap the benefits by using your Biometric screening test results for your Critical Illness health screening! In order to receive your check in the mail, call Aflac toll free at 1-800-433-3036, and inform them you have taken the Fasting Blood Glucose test (which is one of your Biometrics numbers).
