  • What are the Benefits of going to a Healthy Life Center?

    o Conveniently located inside larger facilities to save you time and money.
    o Primary care services.
    o Virtual visits for health plan members with a Healthy Life Center provider in Georgia, Alabama, Mississippi, North Carolina, Oklahoma, South Carolina, Tennessee, Texas, Virginia and Washington. 
    o Sample medications dispensed at no cost, and easy renewal through Express Scripts' mail order program. (May not be available at all locations.)
    o Healthcare information at no cost.
    o Ongoing, quality care from healthcare professionals managed by a regional healthcare leader.
    o Annual DOT physicals are available at these Healthy Life Center locations. To schedule an appointment, text or call the HLC scheduling line (877-365-0051).

     What to Bring to Your Appointment?

    Please bring a valid photo ID and your insurance ID card to your appointment.

    Your health information is protected under HIPAA, and we maintain technical, physical and administrative safeguards to ensure the privacy of your information.

  • Health Plan participants, their covered spouses and covered dependents over the age of 2 may seek care at most Healthy Life Centers (HLCs). HLCs offer a variety of services that includes Primary Care, Acute Care and Chronic Condition Management. In addition, the staff can help with care coordination for referrals, procedures and screenings.

    Acute Care visits include urgent care or minor illness treatments, such as colds, flu, earache, strep throat etc.

    Pediatric care includes non-emergency acute care.

    Primary Care visits have a $25 cost and Acute Care visits have a $20 cost. Wellness, chronic condition management and health coaching are at no cost.

  • Virtual Medicine is the delivery of health-related consultation via a secure kiosk or mobile device. It works like Skype or FaceTime applications. It uses a tabletop Amwell Kiosk to allow for medical equipment to be used during the visit and to connect to a provider located at an existing Healthy Life Center. The services included are the same as the Healthy Life Center except Pediatric Care is for ages 6 and up for non-emergency acute care.

  • Healthy Life Navigators offer support to guide and motivate members to make healthy lifestyle changes. Making lifestyle adjustments is hard on your own, but it’s easier with support. At Mohawk, you don’t have to face your wellness journey alone. They will work with you or your spouse to set a personal health goal, and they will give you the tools to achieve or make progress toward that goal. Healthy Life Navigators build an essential foundation for self-management to help you to continue to use the right tools, behaviors, and resources in day-to-day life. Remember, making small changes makes a big difference over time.

  • With MyChart®, it’s easier than ever to manage your health care.

    Now you can communicate with your Healthy LIfe Center whenever and wherever you like. MyChart makes it convenient and easy. You can use My Chart to schedule appointments, communication with your provider, view test results, request prescription refills and more!

    First, you’ll need to register through a web browser to set up your MyChart account. Then, you can download the MyChart app to access the personalized and secure tool through your smartphone, tablet, laptop or desktop.

    • Fill out the form and complete the questionnaire to verify your identity.
    • Begin the optional two-step verification process.
    • An activation code and instructions will be sent to the email address you provide.
    • Follow the emailed instructions to activate your MyChart account.
