As an enhancement for our medical plan members, we partner with a variety of health-related companies to offer pilot programs. Through pilot programs, we look for innovative ways to help improve member health.
Pilot programs are offered to specific groups based on recommendations from Healthy Life Navigators, Healthy Life Center providers and Healthy Life Care Teams. RecoveryOne has partnered with Cigna to offer Mohawk health plan members access to the RecoveryOne program for muscular pain. The RecoveryOne app, designed by orthopedic specialist, helps make physical recovery easier and more convenient with no physical therapy prescription required.
This online option gives personalized programs for your specific pain or injury. Follow the video exercises and track your progress right in the convenience of your home. Epharmix is a program that automatically monitors health through phone calls and text messages to improve outcomes. It provides alerts and information to Healthy Life Care Teams to offer help and support.
• Diabetes— EpxDiabetes helps patients log their blood sugar levels with reminders via text messages, as well as reminders for ophthalmology appointments.
• Depression—EpxDepression allows for mood charting via text message to have participants rate their mood on a scale of 0 (very sad) to 10 (very happy).
• Hypertension—EpxHypertension helps participants monitor and track blood pressure using text messages and phone call outreach.
Omada is a digital lifestyle behavior change program designed to work with people with pre-emerging health-related issues. Participants receive a wireless smart scale to monitor weight daily, as well as online peer group discussion, weekly interactive lessons and access to a professional health coach. The program works to help members build long-term healthy habits.
