    Welcome to Mohawk benefits!

    Full-time or part-time Mohawk employees scheduled to work 30 hours or more per week are eligible for Mohawk benefits the 1st of the month following 60 days of employment for all plans.

  • Important Biometric Screening & Health Coaching Information

    New employees and spouse/domestic partners enrolled in the Mohawk medical plan must complete a biometric screening within 60 days of your benefit effective date. See Biometrics & Coaching page for details.

    Before You Enroll

    • Review your 2021 benefits on this website and the New Hire Guide
    • Gather your dependent's social security numbers and birth dates
    • Make sure your mailing address and phone numbers are current

    Enroll in Benefits

    Enroll in your benefits in one of two ways:

    1. Online through Employee Central. Click on Change Benefits or View Current Elections tile.

    OR

    2. Call the Benefits Service Center at 866-481-4922 - Monday - Thursday 8 am - 6 pm EST and Friday 8 am - 5 pm EST.

    After You Enroll

    • Review your benefits summary
    • Complete your medical questionnaire(s) (Evidence of Insurability Forms) if necessary

    Note: Medical and vision Open Enrollment for Hawaii residents takes place in June each year. You may not make changes during the Mohawk Open Enrollment event in October

  • Select your pay status below to calculate your costs for medical, health savings account (based on amount to get Company match), dental, vision and accident insurance.

    Don’t forget, Mohawk provides the following at no cost:

    • Basic Life Insurance
    • Basic Accident, Death and Dismemberment Insurance
    • Short-term Disability
    • Employee Assistance (EA)

    And that you are automatically enrolled in:

    • 401(k) at 3%
    • Long-term disability (cost based on pay status)

For 2023 Benefit details, review the pages under the "2023 Benefits" menu above. Also, refer to this page for plan documents and other important information.

All employees hired on or after Jan. 1, 2023 may contribute to an HSA once you become benefit eligible. However, you will not receive any Company money during the first calendar year of your hire date. In 2024, you will be eligible to receive the initial Company money and the employer match as long as you are contributing at the appropriate level to the HSA Plan.  Please see example below:

Example:

John Doe is hired Jan. 28, 2023. He may start contributing to the HSA Plan once he is benefit eligible; however, during the 2023 calendar year he will not receive any company seed or match.  Starting with his first paycheck in 2024, he will receive the 2024 company seed. As long as the employee is contributing to the HSA Plan, he will receive a match per the terms of the 2024 HSA Plan.
