    Amwell telehealth is a faster, easier way to see a doctor. It lets you have video visits with a doctor 24/7/365. It's easy to use, private, and secure. It’s free to enroll and the cost per visit is $20 for acute care visits for Mohawk health plan members and a discounted rate of $59 for Mohawk employees not on the health plan.     Severe Weather Awareness

    Code Red or Code Blue? Know the Difference and How to Respond.

    The spring and summer months can bring unpredictable weather like storms, heavy rainfall and even tornadoes in some parts of the country. Typically, during the month of April Mohawk teams across the country practice evacuation drills to ensure everyone knows how to safely respond to a potential emergency at their facility. In order to maintain social distancing recommendations, these drills have been postponed until later this year.

    In an emergency, seconds count, so it’s important for everyone to know how to safely respond. Make sure you know the following:

    > The emergency exits closest to your workstation

    > Your outdoor assembly area (used for code red)

    > Your indoor assembly area (used for code blue)

  • 2020 Biometrics

    2020 Biometrics

    If you enroll in the Company’s medical plan, you and your covered spouse will be required to complete a biometric screening in 2020.

    At this time, we have had 12 cases at 10 of the company’s more than 450 U.S. facilities. We have localized response plans in place and have communicated directly with impacted individuals. We encourage our employees and their family members to take preventive measures based on the CDC’s guidelines, and have determined it is best to postpone the 2020 Biometric Campaign. We will work with Quest and our HR Partners to reschedule onsite events for a later date but are postponed until further notice.

Taking a few minutes to name your beneficiaries now will help ensure that your plan assets will be distributed according to your direction.

    This service is a straightforward, convenient process that takes just minutes. To access this service:

    You may also contact Fidelity at (800) 835-5087 and request a beneficiary form. Please be sure to review your beneficiaries regularly and update them after certain life events, such as a marriage, divorce, birth of a child, or a death in the family.

    New Hires Biometrics

    As a new hire, if you enroll in the Company medical plan, you and your covered spouse/domestic partner must complete a biometric screening within 60 days of your benefit effective date.

    Employees and covered spouse/domestic partners who do not complete a biometrics screening will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium. Click below for details.

    Qualifying Life Event (QLE) Enrollment

    If you have had a qualifying life event (QLE) (example: birth, marriage, divorce) and need to enroll or make a change to your benefits, you may do so through Employee Central.
