    To download the Spruce app, text 877-365-0051 to get a link or go to www.mymohawkbenefits.com/hlct for a direct link. Once you have downloaded and set up your account, you are ready to text with your Healthy Life Care Team.

    2018 Onsite Flu Shot Campaign

    Mohawk’s annual on-site flu shot program will begin the week of September 10.

    Providing flu shots free of charge is part of our vision to become the healthiest and safest population. Flu shots are highly recommended for most people, particularly for those who have chronic conditions. These vaccinations can help protect employees and those around them throughout the year. We also believe providing flu shots is an important preventive measure that helps ensure we do not see interruption in production due to illness.

    During onsite events, flu shots are offered free of charge to ALL employees, even those who are not on Mohawk’s health plan. No appointment is required during on-site events.

    401k Beneficiary

    Take care of the people who matter most. The beneficiaries you designate will receive payment of benefits provided under the plan in the event of your death. Taking a few minutes to name your beneficiaries now will help ensure that your plan assets will be distributed according to your direction.

    This service is a straightforward, convenient process that takes just minutes. To access this service:

    • Log on to NetBenefits® at www.401k.com
    • Click “Profile
    • Click the “Beneficiaries” link and follow the instructions provided.

     

     

     

    You may also contact Fidelity at (800) 835-5087 and request a beneficiary form. Please be sure to review your beneficiaries regularly and update them after certain life events, such as a marriage, divorce, birth of a child, or a death in the family.

  • New Hires Biometrics

    New Hires Biometrics

    As a new hire, if you enroll in the Company medical plan, you and your covered spouse/domestic partner must complete a biometric screening within 60 days of your benefit effective date.

    Employees and covered spouse/domestic partners who do not complete a biometrics screening will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium. Click below for details.

    Qualifying Life Event (QLE) Enrollment

    If you have had a qualifying life event (QLE) (example: birth, marriage, divorce) and need to enroll or make a change to your benefits, you may do so through Employee Central. Click on Change Benefits or View Current Elections tile. You may also call the Benefits Service Center (BSC) at 1-866-481-4922.

    BSC hours: Mon-Thu 8 am - 6 pm ET and Fri 8 am - 5 pm ET
