Connect with a Healthy Life Navigator (HLN) as a resource for cost and quality information for high costs medical services to help you and your family get the most out of your healthcare dollars.
An HLN can help you find quality in-network surgeons, providers, or hospitals. In addition, you can get cost estimates for high-cost procedures. Some examples are open heart surgery, gallbladder surgery, hernia, spinal fusion, organ transplant, prostatectomy, partial colectomy, pacemaker implant, exploratory laparotomy (stomach) skin graft, amputation, thyroidectomy, artificial limb replacement, hip or knee replacement.
An HLN can also help explain how your health plan works and pays.

Allowance Maximums
Allowance maximums mean that if you elect to have a MAMMOGRAM or COLONOSCOPY that is more than the allowance, you will be responsible for covering the remaining cost. This remaining cost will not apply to your annual deductible or out-of-pocket maximum.
For MRIs and CT Scans, once you have met your deductible, Cigna will pay 80% per procedure up to the allowed amount, then you will be responsible for any remaining cost.

- Preventive Mammogram-3D: $285
- Preventive Colonoscopy: $2,250
- All MRIs: $2,300 (After deductible is met)
- CT Scans: $2,000 (After deductible is met)

Cost for CT Scan: $2,210
Plan Pays: $1,600 (80% of allowable amount of $2,000)

New Hires Biometrics
As a new hire, if you enroll in the Company medical plan, you and your covered spouse must complete a biometric screening within 60 days of your benefit effective date.
Employees and covered spouses who do not complete a biometrics screening will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium. Click below for details.

Qualifying Life Events
If you have had a qualifying life event (QLE)* (example: birth, marriage, divorce) and need to enroll or make a change to your benefits, please call the Benefits Service Center (BSC) at 1-866-481-4922. Required documentation must be submitted to the BSC for the changes to be complete.
*Due to IRS regulations, most Qualifying Life Event changes must be made within 31 days of the event. Please note IRS regulations for birth, adoption or death must be made within 60 days of the event.
BSC hours: Mon-Thu 8 am - 6 pm ET and Fri 8 am - 5 pm ET
Mammogram Screening Schedule
All women should have one screening mammogram between the ages of 35 and 40. After age 40, a screening mammogram is recommended every year. If you have any breast abnormalities, even a small change or concern call your doctor, who may order a diagnostic mammogram. We only do screening mammograms on the mobile unit. If you meet these age guidelines and have not had a mammogram within the calendar year of 2024, and you are a Mohawk Health plan member, you should get a mammogram.

The screening assesses your body mass index, blood pressure, cholesterol, and A1c blood sugar levels. To pass, you must meet all the standards. Medical plan participants and covered spouses who do not complete biometrics testing will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium.

It can let you know if you are at risk for some serious chronic conditions, such as diabetes, heart disease and high blood pressure. Knowing your numbers can help prevent the onset of chronic conditions. It gives you the opportunity to take action to improve your health.

Members not meeting the recommended biometric standards will be required to engage with a Healthy Life Team health coach to avoid a wellness surcharge. Members must meet all biometric standards to be considered passing.

What is a Wellness Surcharge?

Wellness Surcharge
Employees and covered spouses who do not complete biometrics testing, choose not to work with a health coach, or do not make contact with a coach, will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium.
You may receive a wellness surcharge for any of the following:
- No biometrics screening for employee and/or spouse
- No health coaching engagement for employee and/or spouse
So how do you get a wellness surcharge removed?
- Complete a biometrics screening
- Engage with a health coach or Healthy Life Center provider
- Call the Healthy Life Team at 855-566-4295 to begin working towards a goal
Once the completed screening or coaching has been processed, the surcharges will be removed. The surcharge removal process may take up to 60 days.
