Guarantee Issue amounts: Hourly employees - $100,000 Sales, salaried and nonexempt admin clerical employees - $300,000
If you don't enroll when you are first eligible or during the 2018 Open Enrollment Period (Oct. 11-27, 2017), Evidence of Insurability/Statement of Health is required. Any increases to coverage amounts, will also require Evidence of Insurability/Statement of Health
