    Take care of the people who matter most. The beneficiaries you designate will receive payment of benefits provided under the plan in the event of your death. Taking a few minutes to name your beneficiaries now will help ensure that your plan assets will be distributed according to your direction.

    This service is a straightforward, convenient process that takes just minutes. To access this service:

    • Log on to NetBenefits® at www.401k.com
    • Click “Profile
    • Click the “Beneficiaries” link and follow the instructions provided.

     

     

     

    You may also contact Fidelity at (800) 835-5087 and request a beneficiary form. Please be sure to review your beneficiaries regularly and update them after certain life events, such as a marriage, divorce, birth of a child, or a death in the family.

    Memorial’s Mobile Mammogram Coach is Coming in 2019!

    All women should have one screening mammogram between the ages of 35 and 40. After age 40, a screening mammogram is recommended every year. If you have any breast abnormalities, even a small change or concern call your doctor, who may order a diagnostic mammogram. We only do screening mammograms on the mobile unit. If you meet these age guidelines and have not had a mammogram within the calendar year of 2019, and you are a Mohawk Health plan member, you should get a mammogram.

  • New Hires Biometrics

    New Hires Biometrics

    As a new hire, if you enroll in the Company medical plan, you and your covered spouse/domestic partner must complete a biometric screening within 60 days of your benefit effective date.

    Employees and covered spouse/domestic partners who do not complete a biometrics screening will receive a surcharge of an additional $28.85 per week or $125 per month on top of the medical premium. Click below for details.

    Qualifying Life Event (QLE) Enrollment

    If you have had a qualifying life event (QLE) (example: birth, marriage, divorce) and need to enroll or make a change to your benefits, you may do so through Employee Central. Click on Change Benefits or View Current Elections tile. You may also call the Benefits Service Center (BSC) at 1-866-481-4922.

    BSC hours: Mon-Thu 8 am - 6 pm ET and Fri 8 am - 5 pm ET
