Mohawk Health Plan Enhanced Services

As a Mohawk health plan member, you have many enhanced services available to you.

Amwell

Amwell is a faster, easier way to see a doctor.  Visit a board-certified doctor anytime from anywhere through your phone, tablet or computer 24 hours a day, 7 days a week, 365 days a year. Acute care visits cost $20 for Mohawk medical plan participants and their covered dependents or $69 for Mohawk employees not on the medical plan. It is time to rethink the doctor visit. Behavioral Health

Behavioral Health is an important part of your overall well-being. Mohawk is pleased to offer a variety of resources to our employees and their families. We encourage you take advantage of the resources linked here.  The Employee Assistance (EA) helps you and your family members successfully manage life’s challenges. Wednesday Wellness Seminars Take advantage of these weekly webcasts on a variety of behavioral health topics Wednesdays for one hour at 2 pm ET/ 1 pm CT or watch at your convenience with on-demand replay starting 2 hours after the live presentation. Care Teams

Care Teams are provided to give you complete support for your health and well-being. Did you know you have a team of qualified medical professionals dedicate to you and your family members?  They will guide you through your health journey with the tools you need to be an active participant in your care. Care Teams consists of medical providers, a Healthy Life Navigator, a case manager, referral coordinator, registered dieticians, behavioral health support and so much more!

Case management

Case Management is offered as an enhancement to Mohawk health plan members to help provide peace of mind during complex medical conditions. Case managers may call to get to know you or your family member to be able to provide the help needed to help you or your family member manage your health. If the medical condition requires surgery, a case manager may call before and/or after discharge to answer any questions, help with and issues and provide the support and guidance needed during this time.

Employee Assistance (EA) resources for home and work life

Healthy Life Navigators

Healthy Life Navigators offer support to guide and motivate members to make healthy lifestyle changes. Making lifestyle adjustments is hard on your own, but it’s easier with support. At Mohawk, you don’t have to face your wellness journey alone. They will work with you or your spouse to set a personal health goal, and they will give you the tools to achieve or make progress toward that goal. Healthy Life Navigators build an essential foundation for self-management to help you to continue to use the right tools, behaviors, and resources in day-to-day life. Remember, making small changes makes a big difference over time.
