Basic Employee Term Life / AD&D Insurance

Mohawk is pleased to announce the extra $5,000 burial benefit assistance. You are automatically enrolled in these benefits if you work 30 hours or more per week, but you must designate a beneficiary or beneficiaries for them to receive the funds in the event of your death, accidental death or severe injury.

On the first of the month following 60 days of working 30 hours or more per week, Mohawk provides employees Basic Term Life Insurance at no cost with a benefit amount equal to one times your basic annual earnings.

The Company also provides Basic Accidental Death & Dismemberment Insurance equal to the amount of your Basic Life Insurance, at no cost to you. AD&D Insurance pays benefits in the event of death or dismemberment due to an accident. For loss of life, paralysis, loss of limbs, sight, speech or hearing, you or your beneficiaries will receive an amount equal to or a portion of your full AD&D benefit, depending on the extent of the loss. In the case of death, 100% of the benefit is payable to your beneficiary.

You need to designate a beneficiary (or beneficiaries) for your basic term life and any additional term life insurance you elect. You may do so by calling the Benefits Service Center or through Employee Central.

Supplemental Term Life Insurance

You may enroll in Supplemental Term Life Insurance for yourself and your dependents anytime during the year, during Annual Open Enrollment or if you have a Qualifying Life Event (QLE). If you didn't enroll when you are first eligible, Evidence of Insurability/Statement of Health is required. Any increases to coverage amounts, will also require Evidence of Insurability/Statement of Health (EOI).

  • Hourly employees may elect a maximum benefit amount up to the lesser of 5 times their salary or $250,000 in $10,000 increments.
  • Sales, salaried and non exempt admin clerical employees may elect a maximum benefit amount up to the lesser of five times their salary or $500,000, in $10,000 increments. 

Dependents may be covered if you elect coverage for yourself, however it may not exceed 50% of your elected coverage. You may elect coverage for your spouse in increments of $5,000. The maximum benefit for spouse is $50,000 (hourly employees) and $250,000 (sales, salaried, and non-exempt admin clerical employees).  You may also elect coverage for your children. The benefit for dependent children is $10,000 per child. Contributions are based on age and increase each time an employee or spouse enters a new five-year age group.

New Hire - Guaranteed Issue Amounts

As a new hire, you may enroll in the plan without Evidence of Insurability/Statement of Health for the following pay types and up to the guaranteed issue amounts:

Hourly employees - $100,000 

Sales, salaried and non exempt admin clerical employees - $300,000

Spouse - $10,000

Designating Your Beneficiaries

You need to designate a beneficiary (or beneficiaries) for your basic term life and any additional term life insurance you elect. You may do so by calling the Benefits Service Center or through Employee Central.

Do you have a plan in place for your beneficiaries? If you and your spouse both died in an accident, would your minor children (as beneficiaries) have access to the funds they need for living expenses? For college?

If you name your minor children as beneficiaries and do not have specific language in your will and/or a trust in place, the benefit amount could be held until the minors reach age 18. Probate laws vary by state, which can add to the confusion of planning a trust or preparing a will.
